Hubiste de tener cuidado
I answered that the translation is "You should have been careful." However, the correct answer was "You had to be careful." I'm not sure why my answer was not also correct.

Although the literal translation for your answer "you should have been careful" would be "deberías haber tenido cuidado", the phrase with "hubiste de tener cuidado" could also be interpreted as a "should have.." - I've replaced that option with another one so there's no ambiguity. I hope it helps.
