They are different tenses so, for example:
Si prefieres ir al cine, podemos ir.
If you prefer to go to the cinema, we can go.
Si preferirías ir al cine, podemos ir.
If you'd prefer/if you'd rather go to the cinema, we can go.
¿Qué prefieres hacer?
What do you prefer to do?
¿Qué preferirías hacer?
What would you rather do?
We tend to use more the present tense than the conditional though.

You can also start a sentence with "si preferías..." but this is a different tense to "si prefieres". I'm not sure what you are referring to specifically. Can you expand?
