The question (with no other information provided) is "Drive safely!. One of the options is "Conducir con cuidado".There is another lesson on Requests / Commands / instructions where if I have got it right the infinitive is used without the preopisiton "a".. Why doies it not work in this instance?

You're right. There is a lesson that explains that we can use the infinitive for commands. The context for this is more formal as in general instructions to the public, but as there was no clear indication in this test-question that this was not the case I modified the answers so there won't be any confusion now.
