Hola Kristin,

The sentence in the quiz is this:

¿Señor Muguruza, va usted al trabajo ahora?
(Mr. Muguruza, are you going to work now?

If we are using the pronoun "usted" to refer to "you", the verb needs to be conjugated according to this pronoun which uses the same form as él/ella (he/she), not as tú. Therefore we need "va" here, not "vas".
If we hadn't used any formal pronoun, then we would use "vas". For example:

Antonio, are you going to work?
Antonio, ¿vas al trabajo?
I hope this clarified it.

the answer should be in the yo form instead of the tu form

Hola Trini

The answer would be as you said using the yo form, but the quiz is asking for the verb in the question, not the answer.
